About Community Center for Integrated Health
Community Center for Integrated Health (CCIH) is a CARF-accredited and state-licensed outpatient clinic for adults 18 and older. It is part of Recovery Network of Programs (RNP), which has served the Greater Bridgeport community for decades. They aim to provide integrated care, including primary medical care, mental health services, and substance use treatment under one roof.
Integrated and Structured Care
CCIH offers a wide range of onsite services including primary healthcare, mental health counseling, substance use counseling, medication management, peer-recovery support, and 24/7 crisis intervention. The center serves people presenting with mental health concerns or co-occurring mental illness and substance use disorders. Additionally, CCIH partners with other local providers to provide HIV and hepatitis testing, counseling for children, and more.
Individualized Planning That Improves Outcomes
CCIH uses a “No Wrong Door” service delivery model, meaning they aim to offer a broad umbrella of supports so that individuals, especially from the most vulnerable populations, can access the care they need without being redirected from service to service. The center accepts walk-ins, and offers extended hours. The model reflects a broader philosophy of integrated health, combining primary care, behavioral health, and substance use services to improve outcomes for people with complex needs.
