Lyon-Martin Community Health Services

Rooted in the belief that healthcare should be safe, affirming, and accessible, Lyon-Martin Community Health Services provides integrated primary care, mental health, addiction, and specialty services. The clinic serves transgender, nonbinary, gender-nonconforming, and intersex communities, as well as cisgender women, with particular attention to people who have historically faced barriers to healthcare. Primary care includes gender-affirming hormone therapy and monitoring, surgery referrals, and pre- and post-operative care. Clients with substance use concerns can access addiction medicine, along with referrals to detox, residential, inpatient, and other higher levels of care.

Creating Care Around Individual Needs

Centered on self-determination, bodily autonomy, harm reduction, and trauma-informed care, using an informed consent model for gender-affirming services whenever possible. Clients seeking mental health support can access individual therapy, psychiatry, crisis support, and dialectical behavior therapy (DBT). The Body Sovereignty Program brings primary care providers, therapists, and dietitians together for people navigating eating concerns, body image, gender dysphoria, trauma, and weight-related barriers. Through the weekly Snack Lab, clients practice DBT skills for managing emotions, stress, and relationships, while the Food Group helps them explore their relationship with food.

Community Support Beyond Clinical Care

Beyond appointments, the San Francisco clinic creates spaces for connection and practical support. Free programs include Let’s Talk, a drop-in peer group for trans and gender-diverse people, Black & Proud, a peer-led group for Black LGBTQ+ community members, and gender-affirming haircuts on designated days. Outreach teams distribute hygiene, safer-sex, safer-drug-use, and street-sleeping supplies while connecting people with care. New clients start by calling the clinic for an eligibility appointment to address insurance or sliding-scale costs, then attend a primary care appointment to establish care.
