About Family Mosaic Project
Centering support around children, teens, and their families, Family Mosaic Project supports San Francisco clients ages 3–18 who have experienced trauma, have significant mental health needs, or face the risk of out-of-home placement or a higher level of care. The program works to keep children and teens connected to their families, schools, and communities. Each client receives care based on their needs, bringing together clinical case management, therapy, psychiatry, nursing, behavioral support, crisis services, medication support, and care coordination.
Coordinating Care Around Each Child
Through Wraparound care, Family Mosaic Project coordinates mental health, family, school, and community support around each child’s individual needs. Individual, parent, and family therapy help children, teens, and caregivers build coping skills, improve mental health, and strengthen relationships with family members and identified natural supports. Clinical case managers assess each family’s needs, coordinate care, and connect them with additional resources. Depending on the child, teams can also coordinate occupational therapy, respite, tutoring, mentoring, advocacy, psychiatric medication services, and other community programs.
Building Stability Across Daily Life
Family Mosaic Project partners with community agencies and resources throughout San Francisco to promote permanency and stabilization for children and teens. Staff work with families to stabilize home and educational placements, improve functioning within homes, schools, and communities, and strengthen permanent relationships with family members and other natural supports. By coordinating care around each client’s needs and everyday environment, the program works to prevent out-of-home placement and help children and teens avoid the need for a higher level of care.
