Hofunity Behavioral Health

Located on South 21st Drive in Phoenix, Arizona, the center provides adults with residential treatment including dual diagnosis care, mental health services, substance use disorder (SUD) treatment, and independent living support. It serves medically stable residents, including those with co-occurring disorders, cognitive disabilities, or older adults who require integrated behavioral and medical care. Residents stay from 30-90 days, with extensions available based on clinical need.

Road to Stability through Routines

Residents follow consistent routines, with continuous 24/7 supervision from licensed and credentialed staff. Their routine includes individual counseling 1–2 times per week, daily group therapy and psychoeducational sessions, skills training at least five times per week, and AA/NA meetings on- or off-site. The program also provides medication support with psychiatric oversight, life-skills development, structured skill-building activities, and peer support to promote accountability and recovery.

Program Structure and Community Reintegration

The center uses a 4-phase model: adaptation, emotional stabilization, skill building, and continuity of care. Residents progress through these phases while receiving integrated treatment for co-occurring conditions as needed. The center provides relapse prevention, discharge planning, and coordination with community-based providers, primary care, and regional behavioral health authorities. The program prepares residents for independent living and long-term recovery in the community after discharge.
