Located along the Harlem River, Housing Works - 125th Street Community Health Center delivers outpatient medical, addiction, and mental health care for children, teens, and adults at 145 East 125th St. Patients can reach the clinic by the 2, 3, 4, 5, or 6 subway lines at 125th St. They offer primary care, sexual health, substance use disorder (SUD) services, mental health treatment, telehealth, adult gender-affirming care, and supportive-service referrals. Hours run Monday, Wednesday, and Friday from 9 a.m. to 5 p.m.; Tuesday and Thursday from 9 a.m. to 7 p.m.; and Saturday from 9 a.m. to 1 p.m.
Therapy, Medication, and Specialized Care
Clinicians provide 1-on-1 and group therapy, medication management, crisis intervention, and addiction-related medication support. Child and adolescent psychotherapy begins at age 5 and may include play therapy, cognitive behavioral therapy (CBT), trauma-focused CBT, dialectical behavior therapy (DBT), narrative therapy, expressive art techniques, mindfulness, and family collaboration. The REMS-certified clinic also administers Spravato esketamine for qualifying patients with treatment-resistant depression or major depressive disorder. Staff monitor patients for at least two hours after each dose.
Practical Access for All Ages
125th Street Community Health Center service lets patients attend scheduled care while continuing school, work, caregiving, and other daily responsibilities. New pediatric and adult visits last 30 minutes, while follow-up visits last 15 minutes. Some therapy, medication refills, lab review, and pediatric appointments can take place through telehealth. They also accept various insurance plans and offers a discounted sliding-fee schedule based on family size and income.
