Harmonia Collaborative Care combines individual, couples, and family counseling, along with medication management, crisis intervention, and case management. Connecting Older Adults With Practical Support

Harmonia Collaborative Care uses a holistic approach to help older adults stay independent through Senior Care Coordination, which connects them with housing options, assisted living resources, and other needed support. CarePanion services add in-home support with housekeeping, grocery shopping, home safety checks, and companionship. These services give clients practical care and respectful connection while helping them stay safe and more supported at home.

Focused Care For Complex Needs

Harmonia Collaborative Care also offers focused care for clients with more complex needs. Their Health Home Care Coordination helps eligible Medicaid recipients ages 21 and older manage mental health concerns, chronic conditions, medical appointments, transportation, and benefits. Their 14-week Living without Violence program guides men who want to prevent or end harmful behavior using safety and relationship-focused care.
