Since 1993, this nonprofit organization has provided medical and mental health services in Manchester, New Hampshire. Its care teams coordinate physical and behavioral health services under one roof, with financial assistance and language translation available to improve access for families. They operate on Mondays from 8 a.m. to 4 p.m., and on Wednesdays and Fridays from 1 p.m. to 4 p.m.

Comprehensive Care Across Every Stage

Clients receive age-specific care throughout life. Children and teens receive wellness visits and screenings for depression, anxiety, and substance use, while adults can access behavioral health support and psychiatric medication management during primary care visits. Seniors benefit from chronic care management and home visits, and recovery programs offer medication-assisted treatment coordinated by onsite medical staff.

Community-Based Care

Support also extends into local schools and community programs. K-12 students receive counseling and mentorship from community health workers, while the Adverse Childhood Experiences Response Team connects families with trauma-informed care. Partnerships with community organizations provide life-skills training and support services for at-risk youth.
