Oswego County Opportunities’ Mental Health Transitional Living Program serves adults with psychiatric diagnoses who need structured, supportive housing. It offers supervised residences with 24-hour staff, apartment treatment with scheduled visits, and a short-term crisis bed for those in immediate need. Each option helps individuals move toward independent community living.
Personalized Skill-Building and Recovery
The program focuses on helping residents gain life skills and confidence through rehabilitation services. Counselors offer support with medication management, symptom control, substance use services, crisis intervention, and daily living tasks. Each participant works with staff to set personalized goals that support long-term wellness and community reintegration.
Housing Options with Built-In Support
Residents may live in single or shared apartments in Fulton and Oswego, in a staffed group residence, or with a host family in a certified Family Care home.
